Terms:

1. Schedule dates as early as possible.
2. For an unexpected absence (not perviously scheduled), there will be a minimum arrival time of two hours if the day is available.
3. Fee option and pay dates are to be decided prior to first scheduled day.
4. Fee option is either a set dollar amount per hour for an eight hour day (regardless of minimum hours worked) or a set dollar amount per exam performed regardless of hours worked and I leave when exams are completed for the day (exams are scheduled back to back with no down time in between). Per exam billing forms are provided.
5. Emergency call back is discouraged, but if necessary the set dollar amount per hour will be paid for every eight hours of stand by and time and a half will be paid for a minimum of two hours per emergent exam performed.
6. A minimum four hour paid orientation to equipment with a facility employee prior to assignment.
7. Business casual attire will be worn with facility identification when provided.
8. Facility written ultrasound exam protocols available on site prior to coverage.
9. 48 hour notice required to cancel any scheduled day.
10. Two week notice required to terminate scheduled coverage.
11. Invoice is due on receipt and becomes delinquent if not paid in 10 business days (except for preset paydates, payment due as agreed).  Late fees assessed as accrued.
